Our Services

Rocky Ford Family Health Center, LLC is provides compassionate care for those needing preventative services, as well as those with illnesses and injuries. Our expereinced, professional staff takes pride in our ability to meet the needs of our patients. 

 

Office Visits

  • Our staff makes every effort to schedule appointments in a timely manner. If you cannot wait until the next day to see a provider, ask to speak to one of our Medical Assistants. 

  • Please arrive a few minutes before your appointment time to fill out any necessary paperwork. 

  • Bring all of the prescription medications, herbs, vitamins and over-the-counter medications you are currently taking.

  • If you must cancel your appointment please let us know as soon as possible. A $40 fee may be charged for patients who miss an appointment without calling to cancel.

  • Patients who miss three appointment in a twelve month period will be dismissed from the clinic.

 

Prescription Refills

  • We require 72 hours notice if you need a prescription refill. If you take routine medications, please plan ahead so you won't run out of your medicine. For refills, call your pharmacy. They will then contact us for authorization.

  • If you order prescriptions through the mail, plan to reorder at least 2 weeks before your medication runs out. 

  • Periodically, you will be required to schedule an appointment with your provider before a prescription can be renewed. This allows us to make sure the levels of medication you are taking are performing properly.  
     

Improving Patient Care

Doug Miller, the clinic owner, continues to improve patient care by keeping an open mind and the desire for innovative clinic management. Activities Doug is involved with include:

  • Reach My Doctor Diabetic Registry

  • Vaccines For Children program

  • Reach Out and Read, a program designed to increase pediatric literacy

Patient Policies

Details of our Patient Bill of Rights and our privacy policy are available in the clinic information notebook located in the waiting room. We comply with HIPAA requirements to protect you privacy and Federal Non-Discrimination Laws.

 

Emergencies and After Hours

Our health care providers are on call 24 hours a day, 7 days a week. If you need assistance after clinic hours, you may call the clinic (719) 254-7421 to receive recorded instructions on how to contact the on-call provider. In the event of an emergency, call 911 or go to the nearest emergency room.
